Visiting a new doctor can be a little scary, but it doesn't have to be. If you prepare ahead of time, you can make sure your first visit is as easy and stress-free as possible.

South Florida Institute for Wellness & Health in Aventura, Florida welcomes you to our practice. Once you have made your initial appointment, you may pre-register by downloading, printing, and completing the forms below. Please be sure to bring them with you to your first visit, along with any diagnostic films or other test results that may have been ordered or performed by another physician. Completing the forms prior to your visit will help speed up the new patient registration process.

How to Prepare for Your Visit

Arrive early: It is best to arrive 10-15 minutes early before your scheduled appointment to fill out necessary paper work, especially if you are a new patient. For your convenience, most practices have their forms already online, so a patient can complete them beforehand, to save time. 

What to wear: It is important to dress accordingly to the body part you are having trouble with, so it can be easily examined.

Cancelling Appointments: Please cancel at least one day ahead so that your appointment time can be allocated to another patient who is seeking treatment.

What to bring:

  • If you're seeing a doctor for the first time, or you haven’t been in a while, you will want to bring some additional information:
  • Government issued photo ID
  • Insurance card
  • Co-pay: Be prepared to pay your co-pay if your insurance requires it.
  • Copies of all your medical records, scans, etc.
  • If you have seen a physical therapist, please bring a progress letter from the therapist.
  • If you have had surgery elsewhere, please bring a copy of your operation report
  • A list of all medications that you are currently taking and their doses
  • A list of any known drug allergies and the symptoms you may have from taking these medicines
  • Any medical information or reasons for admission to hospital
  • Your claim number if it is a workers’ compensation injury
